SAFETY OF UNDERGROUND PUBLIC TRANSPORT
This paper describes a Metrorail accident that occurred in suburban Maryland, and the environment in the corporate culture that made it likely that this accident would not be averted. The author, from the National Transportation Safety Board, details the actions of five Metro employees who were involved in the accident and relates their actions. She notes that any one of these individuals could have prevented the accident along the way, but they were all too afraid of losing their jobs to speak up. She emphasizes that the management of this company is responsible for creating a corporate culture that will encourage safe transportation operations.
