DEATH BLAMED ON HUMAN ERROR
In September 1994 the Panamanian registered tank vessel MAISTROS I reported that an enclosed life boat had dropped into the water from its stowed position, killing one crew member and injuring another. The subsequent investigation targeted human error as the cause of the casualty.
