LEARN FROM MISTAKES

It has been estimated that up to 80% of all marine casualties have human-related causes. This article reviews three recent casualties: a collision between an offshore supply vessel and a bulk carrier; the grounding of a tank barge and the collision of a passenger ship and a bulk carrier. The human factors involved are indicated and ways to prevent similar occurrences are recommended.
