MEN, SHIPS AND THE SEA

The article begins with a description of three marine accidents: the explosion on the Piper Alpha platform; structural failure on the accommodation platform Alexander Kielland and the capsize of the floating offshore drilling unit Ocean Ranger. The vast majority of marine accidents occur during systems operations. The article discusses the challenges concerning human and organizational elements in design, construction and operation of systems which must be addressed if substantial improvements are to be made in marine safety.
