Optimization, cost- benefit analysis, specifications

Some of the current issues of design of the safety and serviceability provisions in codes and standard specifications are discussed. Outstanding among these is the problem of gross human error, which is not taken into account explicitly in established code rationales, but which accounts for a large proportion of structural failures and is present in many structures. A combination model is suggested as a possible way to account for human error and illustrated by an example (a).
