ABDOMINAL INJURIES IN BELTED CAR OCCUPANTS

ABDOMINALVERLETZUNGEN BEI ANGESCHNALLTEN PKW-INSASSEN

BETWEEN NOVEMBER 1974 AND JUNE 1978, 60 SERIOUSLY INJURED CAR OCCUPANTS WERE TREATED AT THE KIEL UNIVERSITY SURGICAL CLINIC. THESE PEOPLE HAD WORN THREE-POINT BELTS, AND 19 HAD ABDOMINAL INJURIES. CONSIDERABLE INTRA-ABDOMINAL INJURIES CAN OCCUR AT HIGH COLLISION SPEEDS, MAINLY WHEN BELTS ARE LOOSELY WORN, BUT THESE ARE OFTEN DIFFICULT TO DIAGNOSE. CAREFUL SURVEILLANCE OF THESE INJURIES IS THEREFORE REQUIRED. LEGISLATION INTRODUCED ON 1ST JANUARY 1976 MAKING THE USE OF SAFETY BELTS COMPULSORY, AND THE SUBSEQUENT RISE IN THE NUMBERS OF PEOPLE WEARING BELTS, RESULTED NOT ONLY IN A DECREASE IN FATALITIES, BUT ALSO IN AN INCREASE IN INJURIES CAUSED BY THE BELT; THESE INJURIES ARE WELL KNOWN FROM GARRETT AND BRAUNSTEIN'S STUDIES, AMONG 3325 BELTED VEHICLE OCCUPANTS INVOLVED IN ACCIDENTS. THEY FOUND THAT 944 HAD BEEN INJURED; OF THESE 7 HAD INTRA-ABDOMINAL INJURIES, 7 HAD FRACTURES OF THE PELVIS AND 12 FRACTURES OF THE LUMBAR VERTEBRAE. THE TERM "SEATBELT SYNDROME" WAS COINED FOR THESE INJURIES. IN ALL THESE CASES TWO-POINT LAP BELTS WERE USED. THIS TYPE OF BELT IS HARDLY USED NOW IN THE FEDERAL REPUBLIC OF GERMANY SINCE THE THREE-POINT SAFETY BELT CURRENTLY OFFERS THE BEST PROTECTION. IF THE BELT IS FITTED SNUGLY THE OCCUPANT BECOMES FIXED TO THE VEHICLE AND DECELERATES WITH THE VEHICLE. LOOSE BELTS CAN LEAD TO SO-CALLED "SUBMARINING" THROUGH DEFORMATION OF THE SEAT AND SLIPPING OF THE BELT FROM THE THORAX AND HIP; THIS IN TURN MAY LEAD TO A SUDDEN INCREASE IN PRESSURE ON THE ABDOMEN, CAUSING INTRA-ABDOMINAL INJURY.
