IDENTIFICATION OF CRITICAL FAILURES AND COST EFFECTIVE RELIABILITY IMPROVEMENT APPROACHES
This report represents the results of an analysis to identify safety critical items in Cargo, Utility and Attack helicopters. Included in the report is a detailed failure mode analysis which quantitatively identifies critical modes of failure (both hardware and human) whose occurrence (singly or in combination) can cause hazard in flight. The identified failure modes are ranked in terms of criticality. Improvement recommendations are made and a method is developed to assess their cost effectiveness.
