FUEL PIPING FAULT CAUSED SALLY STAR FIRE
The article describes the fire on board the ro/ro passenger ferry SALLY STAR in August 1994. The fire broke out in the main engine room and was caused by the failure of a bolted flange joint on the low pressure fuel system of no 4 main engine, allowing flammable fuel oil vapour to come into contact with part of the engine exhaust system. The contributing factors which influenced the severity and extent of the fire and remedial actions initiated by the owners are discussed.
