HUMAN AND ORGANIZATIONAL ERROR IN OPERATIONS OF MARINE SYSTEMS: OCCIDENTAL PIPER ALPHA
Based on the well-documented case history of the Piper Alpha catastrophe, probability based influence diagrams have been developed for the direct and compounding sequences of events that led up to the initial explosion. This analysis structure has been used to develop a simplified general "template" for high pressure gas operations that preserves the central causative mechanisms, yet does not preserve the unique aspects of the Piper Alpha disaster. This template is used to examine the potential influences of alternative improvements in operations.
