A PHILOSOPHY FOR CONDUCTING METALLURGICAL FAILURE INVESTIGATIONS

Failure investigations are mainly performed with the object of obtaining the knowledge needed to prevent failure in existing similar equipment and to preclude the possibilities of failure in future equipment by modifying the design or the selection of materials. The principal failure mechanisms are listed; there are many possible techniques for investigating them, and these are not discussed here. Instead the author considers the philosophical approach that should be adopted by the investigator in order to ensure that an objective investigation aimed solely at determining the source of failure is undertaken. A number of simple rules that should be applied are listed.
