REPORT ON STANDARD RELOCATION FORMS
The final draft of proposed application and claim forms are presented with a brief explanation of the purpose and use of the form. Form A may be used to give written notice of eligibility and the dollar amounts of entitlement to eligible occupants. It is also used to supply available housing that will meet the needs of the family. The seven other forms provide for the following: claim for residential moving; claim for rent payment and/or purchase down payment; claim for replacement housing payment; claim for business, farm or non-profit moving allowance; application for determination - "In Lieu of" moving allowance; claim for "in lieu of" moving allowance; and the replacement dwelling certification.
