ONE YEAR'S EXPERIENCE OF MAJOR TRAUMA OUTCOME STUDY METHODOLOGY
This article describes research whose aim is to assess the feasibility and validity of the use of the TRISS (trauma score, injury severity score) methodology for forecasting major trauma outcome. 1577 patients admitted with trauma to the accident and emergency department of Northern General Hospital who were admitted for more then 3 days, or admitted to intensive care, or who had died, were included in the study. These included road traffic accidents. The case notes of each patient were reviewed with TRISS methodology to compare expected and observed survival. Information on the patients physiological state on admission (systolic blood pressure, Glasgow Coma Scale, and respiratory rate) was noted as was his or her age, the cause of trauma, anatomical diagnosis, and any complications or errors in management. The information was entered into a microcomputer database. The probability of survival was calculated by scoring each patient on the basis of their most severe injuries, vital signs on admission and age (injury severity score, revised trauma score, and age code respectively). The injuries were coded according to the abreviated injury scale (1985 revision). Most admissions were due to single orthopaedic injury and serious injuries were uncommon. The TRISS methodology matched the observed income for most seriously injured patients except for elderly patients with single orthopaedic injuries, in whom there were major differences between observed and expected outcomes.
