TRAUMATIC NONPENETRATING CARDIAC DISORDERS

The speed of automobile, aircraft, and motor vehicles, and the high incidence of accidents therefrom, have produced traumatic disorders to the human body. There is a high incidence of injury in the cardiovascular system when the human body is exposed to rapid decelerative impact. The heart may be injured by nonpenetrating mechanical impact as the result of direct or indirect forces. The anatomic location of the heart makes it susceptible to injury. The physican may overlook cardiovascular trauma in the treatment of the injured patient. Vehicle plus fortuitous object, victim plus vehicle, visceral deformation--this is the collision triad which describes the physical events which affect an individual in an automobile accident. The application of force to the body induces pathologic change, which may be overlooked because other bodily injury may mask its manifestations. Any traumatized patient, particularly one having evidence of chest injury, must be studied for cardiac injury. The most common symptom in nonpenetrating cardiac trauma is precordial pain. Serial electrocardiograms are indicated daily for at least five days. A critical evaluation of the pathologic physiology and the pathology of the traumatized cardiovascular system is indispensable in the study of cardiac injury. The results of injury must, if possible, be differentiated from pre-existing disorders of congenital, infectious, or degenerative type. The conclusion of this study is that the efficiency of the restraint of the system of belt restraints is important. If appropriately used, restraints could mollify cardiovascular and bodily injury. The object struck by the victim in the course of decelerative trajectory may be of significance. /SRIS/
