The data were purchased from the Centers for Medicaid and Medicare Services (CMS) with specific Data Use Agreement (DUA) requirements in terms of safeguarding the confidentiality of CMS data. Researchers may download the data directly from the CMS website after completing individual requests. Medicaid Analytic eXtract (MAX) outpatient administrative claims data from January 1, 2010, to December 31, 2013, from the state of Washington were analyzed. These data were obtained from the Centers for Medicare and Medicaid Services.
