The report contains a description of the uniform reporting system for the urban mass transit industry designed and tested in Project FARE. It is presented in five volumes. Volume V contains examples of all of the forms used in the system for commuter rail operations. Each form shows a cross reference to the applicable instructions in Volume IV.
