A Study on Characteristics and Improvement Methods of Ambulance Transportation Time to Emergency Medical Facilities in Rural Area

救急医療機関への移動に長時間を要する地域の特性と改善策に関する研究

The purpose of this study is to find improvement methods regarding infrastructures in the area where long time is needed to transport to existing emergency medical facilities on current road network. The areas are identified by the calculation of transportation time by an ambulance car to an emergency medical facility from a small mesh of 1km*1km, the effect of shortening the time by enhancement of road network and the transportation beyond to the outside of the prefecture are calculated. To shorten the time of ambulance transportation, it is the most important to shorten the time from departure to arrival to emergency medical facility. As a result of two simulations, the effect on shortening the time is bigger in the area where transportation time is long, it is cleared the enhancement of road network and the transportation beyond to the outside of the prefecture are more effective improvement method in those areas.本研究では、現状の施設・道路ネットワークを踏まえての、救急医療機関への移動に長時間を要する地域の特性と改善策を研究した。研究では、救急医療機関までの所要時間を1km*1kmの地域メッシュ単位で算定し、全国で長時間を要する地域を特定したうえで、改善策として、道路ネットワークの強化および県外搬送のシミュレーションを行い、搬送時間短縮の効果を試算した。その結果、現場出発から病院到着までの時間について地域の特性に応じた改善策を検討するのが効果的であること、道路ネットワーク強化、県外搬送では、救急医療機関へ長時間かかる地域に対して、特に短縮効果が大きいこと、道路ネットワークの強化と県外搬送では効果のある地域が異なるため両者を有機的に進めていくことが望ましいこと、また、地域メッシュ単位で算定には実態やシミュレーション結果などを詳細に把握することができるという利点があり改善策の選択・決定等の検討において有用であることなどが分かった。
