RISK ASSESSMENT OF ORGANIZATIONAL FACTORS IN AVIATION SYSTEMS

This article explores the role of organizational factors in failures in the aviation system, using systems theory, including the Reason model of accident causation, Nagel's Information-Decision-Action model and Bayesian Belief Networks. It proposes ways to apply organizational approaches to mitigating risk of failure. Included are case studies of accidents that seem to have had organizational causes and suggestions for ways to identify critical organizational elements.
